As a Physician in our PACE program, youll care for older adults with complex needs through relationship-based primary carepartnering with an Interdisciplinary Team and meeting participants where they are (in the center, at home, virtually, or in skilled nursing settings). This is whole-person, value-based care built around dignity, coordination, and helping people stay healthier at home.
What Does Success Look Like As A Physician?
- Lead within a collaborative IDT model: Serve as a member of the IDT, completing assessments, participating in care planning, and reviewing Service Determination Requests.
- Provide longitudinal primary care: Deliver direct primary medical care to PACE participantsincluding histories and physicalswhile actively growing and managing a panel in close collaboration with a Complex Care RN.
- Drive clinical decision-making: Initiate, evaluate, review, and close orders for medications, diagnostics, referrals, and treatments.
- Treat participants across settings: Evaluate and treat participants in the center, at home, virtually, in SNFs, and other settings as needed.
- Coordinate whole-person care: Liaise with Behavioral Health, Pharmacy, and other IDT members to coordinate care thats clinically sound and person-centered.
- Support hospital transitions: Support participants through hospital admissionsoverseeing inpatient management, updating the IDT, leveraging consultants as needed, and supporting safe discharge planning.
- Support SNF transitions: Facilitate nursing home admissions, conduct in-person assessments, coordinate with SNF staff and the IDT, and support safe discharge when appropriate.
- Teach and empower: Educate team members, participants, and caregivers on medical care plans, medications, and next steps.
- Ensure appropriate specialty care: Initiate referrals to specialists and approve return appointments as clinically appropriate.
- Perform procedures within competency: Perform selected in-office procedures for which competencies are met.
- Document clearly and on time: Document all participant interactions, assessments, and action items in the EMR within specified timeframes.
- Support accurate risk adjustment: Engage with documentation specialists for HCC medical coding capture.
- Provide after-hours support: Remotely take after-hours calls triaged by a nurse team on a rotating schedule.
- Grow in value-based care: Opportunity to learn value-based care strategies and how to manage high-need, high-cost populations.
What Does A Successful Profile Look Like?
- A heart for older adults: 1+ year of experience caring for frail and elderly populations.
- Communication that builds trust: Strong interpersonal and communication skills with participants, caregivers, and teammates.
- Independent leadership + true collaboration: Ability to lead independently while also working collaboratively in a team-based setting.
- Medical training foundation: Graduate of an accredited School of Medicine or Osteopathy with 2+ years of experience.
- California readiness: Active, unrestricted MD/DO license in California and CPR certification.
- Residency completed: Successful completion of an ACGME-accredited residency program.
- Education credentials: Masters Degree / Doctorate Degree.
- Board certified: Board Certification in Internal Medicine or Family Practice.
- Geriatrics a plus: Board Certification in Geriatrics preferred.
- Prescribing credentials:State CDS certification and DEA registration.
- Language skills (preferred): English required; Spanish preferred.
- Mobility to meet participants where they are: Proof of a valid CA drivers license, personal transportation,
